A question worth asking

How do you know if your depression is treatment-resistant?

Clinically, depression is called treatment-resistant when at least two antidepressants, each taken at a proper dose for a proper length of time, have failed to give adequate relief. If the following sound familiar, you likely meet the criteria and you have options you have not been offered yet.

    You have taken two or more antidepressants for at least six weeks each, at a real dose, and still feel largely the same.

    Something helped at first and then stopped working, so the dose kept getting raised without much changing.

  3. 3

    The side effects were the reason you stopped, not the depression lifting. Weight, sleep, or libido made the trade not worth it.

    Nobody has ever measured your symptoms with a standardized score, so nobody can actually say whether anything worked.

Yes. It is a recognized clinical category, not a way of saying you are difficult to treat. It simply means at least two adequate antidepressant trials have not produced adequate relief, and it is the specific criterion insurance uses to approve TMS and Spravato.

TMS is FDA-cleared to treat depressive episodes and to decrease anxiety symptoms in adults with major depressive disorder who also have anxiety. It is not cleared as a standalone treatment for an anxiety disorder on its own, which is a distinction we will be straight with you about at your consultation.

TMS is FDA-cleared as an add-on treatment for major depressive disorder in adolescents aged 15 to 21. Whether it is appropriate for a specific teenager is a decision a provider makes after a full evaluation, with the family in the room.

Good. Keep seeing them. We treat the medical side and coordinate with your therapist rather than replacing them. If you do not have one, we will refer you to someone who fits what you are working through.

That is the norm, not the exception. Depression frequently sits on top of untreated ADHD, unresolved trauma, or an anxiety disorder. Treating only the surface layer is why so many people cycle through medications without getting anywhere, which is why the evaluation looks at all of it.
